What is the Nursing Facility Transition Program?
It’s an initiative from the Michigan Department of Community Health to help people needing long-term care in receiving services in their homes. There is a common misconception that when faced with a disabling illness or accident, or after a hospital stay, the only option is a nursing facility.
What will the program do for me?
If you are interested in moving back into the community, a Care Manager can help you look at your options. If you wish to pursue the transition, funding may be available to help you with housing costs, furniture, household goods, utility hook-up, etc.
What if I need help once I get home?
There are programs available to help with your in-home service needs. The MI Choice Program covers many services including personal care, respite, chore services, adult day care, homemaking, home-delivered meals, private duty nursing, community living supports, non-medical transportation, counseling, personal emergency response systems, medical supplies and equipment, environmental modifications, residential services, nursing facility transition services, and a self-determination option that includes fiscal intermediary and goods and services. Some individuals are even able to return home without any additional services. Each situation is unique and the Care Managers will help you sort through the services and supports available to you.
How do I qualify for the program?
If you are currently a Medicaid beneficiary and have been residing in a nursing home for the past six months (some exceptions apply), you may qualify for the Nursing Facility Transition Program. Call one of the offices listed below for more information. To be eligible for the MI Choice Program, you must need a nursing facility level of care and meet financial eligibility rules, but there are other programs that also may help you remain in your home.
