Applications for enrollment into our program are accepted any time during a program year.  However, to be considered for fall program start, you will want to start the process as early as March.

To complete an application for enrollment, contact  the appropriate staff person for the county in which you reside.  This application appointment will last approximately 20 minutes.  You will be asked to bring the following documentation to help determine eligibility for the program:

  • proof of Income (12 months of check stubs, W-2 form, 1040 tax form, etc)
  • child’s birth certificate
  • last four digits of child’s Social Security Number
  • health insurance card
  • any documentation regarding your child’s special needs or disability (if applicable)
  Our selection process…

§         Begins in June.

§         Selection criteria are used to identify children and families with the greatest need for services.

§         Applications are reviewed and considered for selection if eligible.

§         If selected, you will be notified as to what happens next.

Once selected, the following health requirements will need to be met:

  • Your child will need to have a complete medical exam, including hearing and vision screens, blood pressure reading, and hematocrit/hemoglobin.
  • Your child will need to have a dental examination.  You will schedule this with your family dentist.
  • Your child must be up to date with any needed immunizations.

You will be notified of any program start-up activities, such as open houses, teacher home visits, etc, during the month of August!

If your child is eligible to participate but we do not have any available slots, you will be notified that your child has been placed on the waiting list should openings occur.  If your child is not eligible for the program, you will be notified of this and what the reasons for ineligibility are.

Applications are valid for one year only.  If your child is age eligible for the next program year and you would like to try again, please make another contact.
