Tailored Caregiver Assessment and Referral Program (TCARE®)

TCARE® is a proven program that was specifically designed to support family members who are providing care for relatives who need assistance due to illness or disabilities. Caring for an ill parent, spouse, partner or friend involves much more than physical and emotional support. Caregiving is a life-changing experience that can be both gratifying and stressful. TCARE® is an evidence-based approach that offers providers an affordable program to support family members as they provide care to their loved ones. We have trained teams that can help you determine what services and supports will best suit your needs.

Adult Day Care Centers

Adult Day Care Centers are designed to provide care and companionship for older adults who need assistance or supervision during the day. Programs offer relief to family members and caregivers, allowing them to go to work, handle personal business, or just relax while knowing their relative is well cared for and safe. Kinship Care

Kinship Care is a program for relative care providers caring for related children. If you are age 55 or older and caring for a related child under the age of 18, this program may be able to help you with expenses such as recreational activities, school supplies, clothing, needed furniture and more. Funding is limited. Contact your local Council/Commission on Aging for more information.

Creating Confident Caregivers (CCC)

If you’re a caregiver caring for someone with Alzheimer’s Disease, dementia, or memory loss, this 6 week program is for you. Learn about dementia and its effect on the brain, how to reduce your stress and create a positive environment for your loved one. You’ll also learn how to plan activities for your loved one that are based on their strengths, gain self-confidence in your caregiving role and so much more. Powerful Tools for Caregivers
