A Matter of Balance
Eight two-hour small group sessions led by a trained facilitation that helps older adults who are concerned about falls; have had falls in the past; restrict activities because of a fear of falling OR are interested in improving flexibility, balance and strength. Personal Action Toward Health (PATH)
PATH empowers people to take an active role in managing a chronic or long-term health condition. Workshops meet for 6 weeks in 2½ hour sessions. Adults with any chronic condition such as arthritis, heart disease, emphysema, depression, etc. benefit from creating personal action plans to achieve practical goals in managing their conditions.

Personal Action Toward Health-Diabetes (Diabetes PATH)
Like PATH, Diabetes PATH is a 6 week 2½ hour workshop specifically designed for adults with Type 2 Diabetes. Diabetes PATH is a complimentary program to the diabetes education you receive from your health care provider. Diabetes PATH helps you learn techniques to take charge of your diabetes, better communicate with your health care team and make weekly action plans to help you stay on track. Personal Action Toward Health-Pain (Pain PATH)
Is a self-management program that will not conflict with your existing pain management programs or treatment. Instead, it will give you the skills to coordinate all the things needed to manage your health and pain, and help you keep active. Workshops 2½ hours for a six week period. Creating Confident Caregivers (CCC)
If you’re a caregiver caring for someone with Alzheimer’s Disease, dementia, or memory loss, this 6 week program is for you. Learn about dementia and its effect on the brain, how to reduce your stress and create a positive environment for your loved one. You’ll also learn how to plan activities for your loved one that are based on their strengths, gain self-confidence in your caregiving role and so much more. Elder Abuse Prevention
