Person-Centered Options Counseling

Person-Centered Options Counseling (PCOC)

Person-Centered Options Counseling (PCOC) is an interactive, unbiased, decision-support process that helps individuals make informed choices about long-term services and supports, focusing on their preferences, strengths, and values. PCOC assists individuals on how to best pair personal resources with home and community-based services and resources. Whether you are looking for services to stay in your home and live independently or options for assisted living and nursing home care, this program can help you learn about what is available for you in your community.

Benefits of Person-Centered Options Counseling

Through PCOC you share your goals, hopes, interests, dreams, and preferences for your life. You will plan how you will work to meet those goals in your plan, including the services and supports you need.
