MI Choice Waiver Program
MI Choice is a program that provides an array of Medicaid-covered nursing home services to anyone age 18 or older so that they can remain in their home with the same level of service. To qualify for the program, you must be:
- Medically eligible for nursing home care AND
- Financially eligible for Medicaid. For 2023 that means income at or below $2,829.00 a month and no more than $2,000 of countable assets.
This program may provide some of the following services:
- Adult Day Centers
- Chore Services
- Community Living Supports
- Counseling
- Environmental Modifications
- Fiscal Intermediary
- Goods and Services
- Home Delivered Meals
- Medical Transportation
- Medical supplies and equipment not covered under the Medicaid State Plan
- Non-medical Transportation
- Nursing Facility Transitions (NFT)
- Nursing Services (preventative nursing)
- Personal Emergency Response Systems
- Private Duty Nursing
- Respite Services
- Supports Coordination
- Training in a variety of independent living skills
